• All businesses must provide an employer identification number (EIN) and physical & mailing address. Beneficial ownership of 25% or more will require each signer to provide a social security number, physical address, and date of birth. 

Corporation

• Signed Resolution adopted by the Corporation's Board of Directors (add Bylaws before signed resolution of signed minutes designating authorized signers)
• Articles of Incorporation and Certificate of Good Standing from Secretary of State

Limited Liability Company (LLC) 

• Articles of Organization and Certificate of Good Standing from Secretary of State
• Organization Resolution or Operating Agreement (signed)
• Trade Name Affidavit (if operating under a different name)
